What should I bring to my appointment?
A valid referral from your GP or Specialist
Medicare card and/or pension card
Up-to-date list of medications
Any relevant tests including: recent blood test results, blood glucose record/glucometer and relevant scans
What if I want to reschedule or cancel my appointment?
Cancelling or rescheduling an appointment must be done at least 24 hours prior to the appointment. Please contact the relevant reception staff to do so.
I need a commercial drivers license completed. What do I need to bring?
In addition to the above, please bring:
A recent eye report from an optometrist/ophthalmologist
Your blood glucose diary with 5-7 days of documented glucose readings
Blood tests no older than 3 months.
What are the consultation fees?
Consultation fees will be outlined at the time of booking. A valid referral letter and Medicare card is required to be eligible for a Medicare rebate.
